pain of a child-mom

In rural Madagascar, it is not uncommon to see child-moms and children caring for children. I saw this child mom carrying one child on her back, looks like pregnant with one,and carrying a bucket of yams to make a meal for the day. I am not sure what the little boy is doing, intimidating her or just cracking a joke!! Tha pain of the mom is written on her face
